Navigating the Kentucky BioTech Facility

The mission begins with Case and Marshall entering a Kentucky BioTech facility filled with toxic gas, necessitating gas masks. An elevator malfunction results in a fall and gas mask breakage, triggering hallucinations. The player regains control in a chaotic environment.

Locating the Security Desk

Explore the crash site until you find a locked red-lit door. Use a hatchet found on a mannequin to pry it open. Proceed through the hallway, up the stairs, and to the central elevator. Activating the elevator unleashes hallucinatory zombie mannequins; eliminate them with the hatchet. A phone call at the circular desk directs you to the biotechnology room, requiring four director's cards (Red, Green, Blue, Yellow). You'll receive a map to the Yellow card.

Acquiring the Yellow Card and Grappling Hook

From the security console, follow the map to a yellow staircase leading to the Director's office. Solve the computer puzzle ("Access" and "Lift") to access the A.C.R. room. More axe-wielding zombies appear.

After eliminating them, a mannequin holding the Yellow card transforms into an abomination. Before engaging, gather armor plates, weapons, and the crucial grappling hook from the surrounding area. Use tactical explosives (C4 or grenades) to quickly defeat the abomination and its zombie horde. Retrieve the Yellow Card.

Obtaining the Green Card

Use the grappling hook to ascend to the main facility. Next, acquire the Green Card from the Administration Facility. Grapple to the facility from the security desk. Answer the phone and locate four documents to place in the file display area. Mannequins will pursue you; maintain distance by sprinting. Document locations: a desk in the corner, near a round table, on a small central table, and in the cafe near a sink. After placing the documents, grapple the attacking red mannequin repeatedly until it transforms into a Mangler Zombie. Defeat it to obtain the Green Card.

Securing the Blue Card

Grapple across to the Joint Projects Facility. Answer the phone, then locate the Blue Card in a glass chamber surrounded by camera stands. Eliminate the spawning mimic by shooting moving objects to reveal it.

Retrieving the Red Card

Head to the East Wing, following the red carpets to a room with water, a console, and a Mangler. Interact with the console to reveal the Red Card. Grapple to the upper area, swim through the red tunnel, climb the ladders, and eliminate spawning zombies. Unlock a door using a blacklight-revealed code. Activate a timer and turn all drain switches within 25 seconds (one in the starting room, one in the unlocked room, and one accessible via grappling hook). After draining the water, follow the Mangler's path and defeat it along with its zombie horde to obtain the Red Card.

Confronting the Disciple

Return to the security desk and insert all four cards. Take the elevator to the top floor. Answer the red phone to trigger a cinematic leading to a confrontation with the Disciple and a horde of zombies. Completing this encounter reveals the entire sequence as a hallucination.
